Re-ablement and Value for Money: The Promoting Independence Team
As Head of Service, Physical and Sensory Impairment, I created a re-ablement service, the Promoting Independence Team, as the 'front door' of the service. Unlike most reablement services, this service only worked with people under 65, but
Developing the team took 18 months, and involved joint working between Social Workers, Occupational Therapists, Commissioning Officers, the Social Care Development Centre and care agencies from the private sector. The service was underpinned by the Therapy Outcome Measures model which supported us to evidence:
Initially the service was offered just to people newly referred to the service but as expertise in the team developed it began to be used in reviews of long term support packages.
In 2008 the Promoting Independence Team won the Community Care Excellence Network award for best new model of service delivery (see 20:47 on the video below)
- delivered improved outcomes for individuals
- delivered significant cashable savings
- reduced the number of people who needed support.
