Programme Lead for the Assessment & Care Coordination Review Dorset
In response to the Care Act, Dorset's Pathways to Independence programme is developing separate Early Help and Long Term locality based services. My role was to support the review of the 'field work' teams, supporting Dorset to understand which resources to move across the the Early Help service and how to change the locality teams to complement both the Early Help model and to continue the journey towards integration with health under the 'Better Together' programme (the pan-Dorset Better Care Fund response).
I developed project tools to deliver a process of 'co-creation' of this new model with front line staff, with the aim of delivering real cultural change so as deliver core objectives of the project:
